INSTRUCTIONS:  

In order for us to be sure you have received, signed and dated any of these forms applicable to your time with us correctly, please follow these detailed steps to ensure success for all parties.  *Please keep in mind that each person in your party will need to fill out any form applicable to their time and/or stay with us due to insurance reasons.  Thank you for understanding!

1. Click the below link(s) and you will be routed to Adobe Sign.
2. Follow the instructions in order to fill out, sign and date required forms.
3. Adobe Sign will need to confirm your email address; so please be sure to check your email after submitting the form to confirm your signature(s).  *You must confirm your email address; form signature in order for your form to be held true and valid.
4. You will receive signed form(s) and so will we.  
5. The *Medical Authorization form must be printed and signed offline; brought with you.
